The function of the practice manager.

Several GP practice support workers (poh) work in the practice. A poh monitors and guides patients in specific areas of interest and works in close consultation with the GP. There are different specialisations: somatic, elderly and mental health (ggz).

Practice counsellor somatic

In our practice, the poh somatics supports patients with chronic conditions, such as diabetes, cardiovascular diseases, lung diseases (such as COPD and asthma) and elderly people with complex care issues. The poh somatics:

  • Performs checks (e.g. blood pressure and lung function).
  • Provides education on conditions, medication and lifestyle.
  • Provides guidance and establishes a treatment plan with you.
  • Works closely with GP and other healthcare providers.

When you come under the treatment of the poh somatics, a periodic appointment is usually scheduled. During this appointment, we will discuss your care needs and how we can best support you. The somatic physician keeps in regular contact with you and coordinates everything with the GP.

Practice support officer for the elderly

The poh elderly supports elderly people with complex care needs. This caregiver focuses on promoting independence, preventing deterioration and coordinating care. The poh elderly:

  • Conducts interviews to identify care needs and well-being.
  • Counsels on chronic conditions and medication use.
  • Consults with the GP and engages other healthcare providers if necessary.
  • Advises on adaptations that enable longer independent living.

Practice counsellor ggz

The poh-ggz offers support for psychological complaints. These may include stress, anxiety, gloominess, sleep problems or other mental challenges. If necessary, the GP can refer you to the poh-ggz. The poh-ggz:

  • Conducts interviews to explore complaints and causes.
  • Provides advice and support on coping with mental health issues.
  • Helps with practical tips or lifestyle advice.
  • Refers, if necessary and in consultation with your GP, to specialised care.
