Several GP practice support workers (poh) work in the practice. A poh monitors and guides patients in specific areas of interest and works in close consultation with the GP. There are different specialisations: somatic, elderly and mental health (ggz).

When you come under the treatment of the poh somatics, a periodic appointment is usually scheduled. During this appointment, we will discuss your care needs and how we can best support you. The somatic physician keeps in regular contact with you and coordinates everything with the GP.
