For a change of address within the following areas, please submit the form below:

• Ouderkerk aan de Amstel (postcode area 1191)
• Nes aan de Amstel (postal code areas 1188 and 1189)
• The parts of Amstelveen bordering Ouderkerk aan de Amstel (postal code areas 1183, 1184 and 1185)
• Abcoude, for residents at Voetangelweg or Botshol (postal code area 1391)

Are you moving to a postcode outside the above areas? If so, we recommend finding a new GP in your area. This is important because of the possibilities for a home visit and/or quick help in emergencies. Once you have found a new GP, please let us know by contacting the practice.
