Within our practice we work with pairs of GPs. This means you can register with a team of two GPs. Currently, you can register with one of these practices:
Have you had a baby? Then don't forget to register your baby with us. This does not happen automatically. You can easily register your baby with your own GP using the registration form. Is the practice where you are registered currently closed for new registrations? In that case, tick 'no preference' on the form. This way, your child will still be registered with your GP.
Would you like to register several family members? Children under 16 years old can be registered via the form of one of the parents; a separate form is not required. A separate form must be submitted for children aged 16 and above.
Huisartsenpraktijk Aemstelwijck cannot register patients from Villa Ouderkerk. Villa Ouderkerk admits people with an indication for the Long-Term Care Act (WLZ). Inpatient care requires specialised expertise from a team around a specialist in geriatrics. You can discuss with the Villa Ouderkerk organisation how medical care for you or your relative will be arranged.
Do you live in the area where we provide care? If so, you can enter your details here. After you have registered, you will also be registered for the patient portal. You will receive an e-mail from this, which serves as confirmation of registration.
Note: Children under 16 years of age can be registered through one parent's form; no need to submit their own form.
